Enhanced Family Care

Individuals with developmental disabilities and acquire brain injuries are matched with amazing foster care providers. Individuals are welcomed into the caregiver's home and become an active member of the provider's social circle. Enhanced Family Care (EFC) is an effective support system that provides for an inclusive environment for the individuals to learn life skills, engage in community activities and grow in his or her new home.

Respite Services

We provide respite services to relieve our foster care providers and our individuals who need a break to refuel and recharge.

Nursing Services

We take the health of individuals very seriously and thus our nursing staff oversee and manage all the individuals we serve. Our experienced state certified nurses conduct periodic assessments in-home at the caregiver's residence. Some of the things our nursing staff provide to our providers in service to our individuals are offering

  • Consultative services 24/7 to our providers
  • Oversee the proper medication administration and logging of medical data coordinate with individual's team and other licensed medical professional(s) on any medical services needed by individual(s)
  • Educate individuals who want more independent and have the desire to self- administer their own medication.

Crisis Intervention Support

On occasion, individuals may find themselves in potentially disruptive situations that may result in them being displaced from their homes. Since every event is different, we carefully assess the situation and provide the most appropriate level of support for the individual served.

We at HPN provide judgement-free crisis intervention in conjunction with other service providers for a variety of situations that may arise at the individual’s home. Support can range from in-home crisis support to short-term emergency respite, referral for advanced inpatient support care or even transition into a new placement.
