Our Shared Living – Enhanced Family Care program offers supportive home environments for individuals who benefit from residential support while maintaining choice, independence, and meaningful involvement in their everyday lives. Each living arrangement is centered around the individual’s needs, preferences, routines, goals, and desired home environment.

Individuals and families are involved in identifying and selecting a residential provider and household that is a good fit. Depending on the individual’s preferences and support needs, options include, but are not limited to, a traditional family household with parent(s), children and/or other family members; a household with two approved residential providers who share responsibility for providing support; or a roommate-style arrangement with a single approved residential provider.

Residential providers offer a safe and supportive home while providing individualized assistance in accordance with the individual’s service plan. We provide ongoing oversight and support to both the individual and residential provider to help ensure that services remain appropriate as needs and circumstances change.

Our goal is to create a stable home environment that supports independence, personal choice, community involvement, and the individual’s ability to live as independently as possible.

Temporary supports designed to provide relief to caregivers while ensuring individuals continue to receive safe, person-centered support in a stable and supportive environment.

Nursing Support provides clinical oversight and consultative guidance designed to promote health, safety, and quality of care for individuals served in our programs. Through assessments, caregiver support, medication oversight, and coordination with healthcare professionals, these supports help promote safe and person-centered service delivery.

Support may include:

• 24/7 consultative support for caregivers
• Guidance and oversight related to medication administration and health monitoring
• Coordination and communication with medical providers regarding healthcare needs
• Education and support for individuals seeking greater independence, including medication self-management

Crisis Intervention Support provides responsive, short-term supports intended to help address unexpected situations that may affect an individual’s stability, health, safety, or continued success in their home environment. These supports may assist with de-escalation, problem-solving, coordination, and stabilization when urgent support needs arise.

We provide home and community-based services to adults seeking support to live independently and actively participate in their communities. Our services are tailored to each individual’s unique needs and goals, providing the right level of support at every stage.

Services may be paid for through:

• Medicaid Waiver Programs: Available to eligible individuals through New Hampshire’s home and community-based services programs. Contact us and we’ll help connect you with your local designated Area Agency to determine eligibility and next steps.

HCBS services are currently available. We are actively accepting referrals and working with individuals and families to begin services based on availability and service needs.

New Hampshire Medicaid Provider
HP Network is an enrolled New Hampshire Medicaid provider authorized to provide applicable Home and Community-Based Services (HCBS) to eligible individuals through New Hampshire’s developmental services system.
