We’re more than just a provider agency—we’re a team of dedicated professionals passionate about making a meaningful difference in the lives of the people we serve. Based in New Hampshire, HP Network provides person-centered home and community-based services designed to help adults maintain independence, build skills, and live fulfilling lives in their homes and communities.
Our services include support for adults with developmental disabilities and acquired brain disorders through New Hampshire’s home and community-based services system, as well as non-medical Homemaker services for adults who need assistance maintaining their homes and managing everyday household activities.
At HP Network, we believe everyone deserves the opportunity to live with independence, dignity, choice, and the support they need to thrive. We work closely with each individual, their family, and other members of their support team, when applicable, to provide services that reflect the individual’s needs, goals, preferences, and circumstances.
What makes our approach different is our commitment to person-centered support. We understand that no two people are the same. We take the time to listen, understand what matters to each person, and provide support that respects their choices and promotes independence.
For individuals receiving developmental disability or acquired brain disorder services, our goal is to help them develop and maintain skills, participate meaningfully in their communities, and live as independently as possible.
Through our non-medical Homemaker services, we also assist adults who may need help with household activities such as light housekeeping, laundry, meal preparation, grocery shopping, errands, and other approved instrumental activities of daily living. These services are designed to help individuals maintain a safe, clean, and comfortable home environment while preserving their independence.
At HP Network, our mission goes beyond completing tasks or providing services. We want the people we support to have greater choice, confidence, independence, and opportunity in their everyday lives.
