Shared Living

Our Shared Living program offers individuals supportive home environments designed to promote independence, stability, and meaningful community inclusion through several model options, including:

    • Enhanced Family Care (EFC): Family-based shared living within a supportive family household.

    • PartnerCare Support: Shared living supported jointly by two approved co-providers in a stable household.

    • CompanionCare Living: Shared living in a companion-style home setting with a single approved provider.

Respite Support

Temporary supports designed to provide relief to caregivers while ensuring individuals continue to receive safe, person-centered support in a stable and supportive environment.

Nursing Support

Nursing Support provides clinical oversight and consultative guidance designed to promote health, safety, and quality of care for individuals served in our programs. Through assessments, caregiver support, medication oversight, and coordination with healthcare professionals, these supports help promote safe and person-centered service delivery.

Support may include:

• 24/7 consultative support for caregivers
• Guidance and oversight related to medication administration and health monitoring
• Coordination and communication with medical providers regarding healthcare needs
• Education and support for individuals seeking greater independence, including medication self-management

Crisis Intervention Support

Crisis Intervention Support provides responsive, short-term supports intended to help address unexpected situations that may affect an individual’s stability, health, safety, or continued success in their home environment. These supports may assist with de-escalation, problem-solving, coordination, and stabilization when urgent support needs arise.
