Individuals with developmental disabilities and/or acquire brain injuries are matched with amazing home care providers. Individuals are welcomed into the caregiver’s home and become an active member of the provider’s social circle. Enhanced Family Care (EFC) is an effective support system that provides for an inclusive environment for the individuals to learn life skills, engage in community activities and grow in his or her new home.

We provide respite services to relieve our foster care providers and our individuals who need a break to refuel and recharge.

We take the health of individuals very seriously and thus our nursing staff oversee and manage all the individuals we serve. Our experienced state certified nurses conduct periodic assessments in-home at the caregiver’s residence. Some of the things our nursing staff provide to our providers in service to our individuals are offering

• Consultative services 24/7 to our providers
• Oversee the proper medication administration and logging of medical data coordinate with individual’s team and other licensed medical professional(s) on any medical services needed by individual(s)
• Educate individuals who want more independent and have the desire to self- administer their own medication.

Occasionally, individuals with disabilities may find themselves in disruptive situations that may result in dysregulation in their community. These unfortunate situations without the appropriate mitigating solutions and the proper timing response from the individual’s team may result into a bigger crisis. Crisis can stem during day and/or residential services hours.

HPN Managers in charge of running those respective services carefully assess the situation upon receiving notice and provide the most appropriate level of support towards mitigating the situation from becoming a Crisis in the first place for the individuals served. We accomplish this by ensuring ongoing behavioral and clinical support necesary for the individuals well-being and also staying in constant communication with our Area Agency partners and other stakeholders.

To ensure that our individual’s served are both engaged in their communities as much as in their homes HPN offers community-based and supported employment services, and/or engaging volunteering opportunities, thus fostering an environment where clients become integral members of their respective communities outside their residential setting.
