Adult Home & Community-based Services
Roommate Care Support (RCS)
Roommate Care Support pairs an individual with a disability in need of care with a live-in caregiver who provides personalized support in a shared home environment. This service fosters a sense of independence, similar to family-based care, but with a more relaxed, college-style roommate dynamic. The individual has many options when it comes to choosing a caregiver, including selecting one based on shared interests or preferences. For example, they may choose a caregiver within their age range or someone older, depending on what feels most comfortable for them.
This service combines professional care with the benefits of a live-in roommate, assisting with daily activities while promoting independence. It’s a great option for those who prefer a roommate-style living arrangement over traditional family-based care, all within the comfort of a home setting.
Enhanced Family Care (EFC)
We believe that everyone deserves to feel at home and be part of a community. In our Enhanced Family Care program, individuals with disabilities are matched with compassionate caregivers who open their homes and hearts. This program allows individuals to live in a welcoming environment where they become an active part of the family. It’s not just about care—it’s about building relationships, learning life skills, engaging in fun community activities, and growing in a safe, supportive home setting.
Respite Services
Taking care of others can be rewarding, but also exhausting. Our respite services offer a much-needed break for both caregivers and the individuals they care for. Whether it’s for a few hours or a longer period, we provide a safe and supportive environment, allowing caregivers to recharge while ensuring that individuals continue to receive the care and attention they need. This service is vital to helping caregivers maintain their well-being so they can continue to provide the best care possible.
Assistive Technology Services
We provide a range of assistive technology solutions designed to help adults with disabilities live more independently and improve their quality of life. Our services include:
- Personalized Assessments: We start with an in-depth evaluation to understand your specific needs and recommend the right technology for you.
- Assistive Devices & Software: From screen readers and speech-to-text tools to adaptive keyboards and communication aids, we offer a variety of options to support different challenges.

Nursing Services
At HP Network, we take health and safety seriously. Our skilled, state-certified nursing staff ensures that the individuals we serve are well taken care of. We conduct regular in-home assessments to monitor health, administer medications, and coordinate with other healthcare professionals when necessary. Our nurses offer:
- 24/7 consultative support for caregivers
- Supervision and guidance on proper medication administration
- Ongoing communication with medical teams to ensure any healthcare needs are addressed
- Education for individuals who wish to become more independent, particularly around medication self-management
Crisis Intervention Support
Sometimes, life throws unexpected challenges our way, and people may find themselves in urgent situations where they need temporary support. Whether it’s due to being displaced from their home or facing a crisis, HP Network (HPN) is here to help when there are no other options available. We provide short-term care to ensure both the individual’s safety and the safety of those around them, without passing judgment on the circumstances that led to the crisis.
During these tough times, HPN offers emergency respite care, where we assess the individual’s needs—whether physical, behavioral, or psychological. These assessments help our team figure out the best next steps for care and support. We work to provide this support within the person’s current home environment, whenever possible, and with the consent of the individual (or their guardian/case manager). While these services are temporary, they give people the immediate relief they need while ensuring they have a plan for continued care.
